Your first visit

New patients are warmly welcomed at Imagine Advanced Dental Arts. Your first appointment includes a comprehensive evaluation, X-rays as needed, and a personal conversation about your goals. Please plan to arrive 15 minutes early to complete paperwork — or download the forms below to fill out at home.

Insurance & financing

We are happy to help you understand and maximize your dental benefits. Our office accepts most major insurance plans and offers an in-house Membership Plan for patients without dental insurance — including discounts on cleanings, exams, and select treatments.
